All business issues are resolved using SDR: Surface, Diagnose, Resolve.
This page holds the rules. The live list of open issues is kept in the Issues Register itself and worked every week at the Weekly Leadership Sync.
Any issue that cannot be resolved in one Weekly Leadership Sync goes on the register.
Issues are not complaints. They are opportunities to permanently fix what is holding the business back, not to patch it temporarily.
Replaces EOS IDS.
| Step | What to do | |

| 1 | Surface | Name the real issue, not the symptom. Be specific about what is actually wrong and why it matters to the business. |
| 2 | Diagnose | Discuss at root-cause level. What is actually causing this? Don't jump to solutions before the cause is clear. |
| 3 | Resolve | Agree a solution. Assign one action item to one person with a due date. Move on. No revisiting next week unless the action is incomplete. |
An issue is added to the register when any of these happen:

The live issue list changes every week and can name seats under review, so it is kept in the register itself, not on the wiki. Seat-fit concerns are handled in the Seat Transition Plan (leadership only); see Seat Fit Assessment.
